This is a very difficult situation for everyone involved. You are right when you say that a doctor must cause pain some times, in order to heal their patient, and in this case I don't think Draco will be able to move forwards unless he goes through the pain of actually "losing" his hopes about the possibility of building a romantic relationship with Severus.
